Designed as deliberate practice in the language discrimination behind every note that survives review: does this line record what happened, or what an adult concluded? Read each entry as written and make the call.

The categories

Records what happened — Actions, words, counts, and times a camera would capture. Two observers would write it the same way, and a reader can picture the moment.
Records a conclusion — A label, judgment, or guess about what's inside the student's head — motive, feeling, diagnosis, or a summary word standing in for what happened.
